The Clinical Support Specialist serves as an on-site subject matter expert and clinical resource for Care Managers. The Clinical Support Specialist provides support to Care Managers as they deliver comprehensive care management for individuals with intellectual and developmental disabilities using a person-centered planning process. With the support of LIFEPlan’s Program division, the Clinical Support Specialist, will serve as a resource for Care Managers as they conduct comprehensive health and behavioral health assessments to create Individualized Life Plans. The Clinical Support Specialist will provide support and guidance to Care Managers with a focus on building their capacity to coordinate care for health and behavioral health needs. This assistance will encompass areas of assessment, goal selection, interventions, preventative care, health and wellness, and community supports as they pertain to health and/or behavioral health concerns the member has identified. The Clinical Support Specialist will be closely familiar with evidence-based wellness, prevention, and treatment practices for a variety of health and behavioral health disorders in order to achieve optimal outcomes for members. The Clinical Support Specialist will also have a comprehensive understanding of Person-Centered Planning and Health Home Care Management services. The Clinical Support Specialist will be expected to provide direct consultation to Care Managers and Supervisors, under the supervision of the Clinical Director and with support from other members of the clinical team, on individual member cases that present the highest level of risk and complexity.
Bachelor’s degree in behavioral health field (e.g., social work, psychology)
Licensed Practical Nurse or Registered Nurse
Minimum of two or more years of relevant experience:
Experience in health or behavioral health case management is strongly preferred.
Experience with Developmental Disabilities population strongly preferred.
Experience with training, consultation, and assessment in the DD or Mental Health field strongly preferred.
Knowledge of NYS Mental Health system and/or OPWDD system preferred.
Excellent written and verbal communication skills.
Reasoning Ability: Clinical Support Specialists must have the ability to apply relevant state/federal regulations to a variety of circumstances - e.g., NYS Department of Health – Health Home, OPWDD Regulations: 624, 633, and 635, and the Justice Center regulation 625.
Demonstrated ability to take initiative and see a project through to completion
Demonstrated project management skills appropriate to manage multiple initiatives at one time
Minimum intermediate-level Microsoft Office skills, and demonstrated ability to learn database programs and Health Information systems such as Medisked, etc.
